What to Bring to Your Appointment

Completed New Patient Packet (available by clicking Forms above)
- Drivers License
- Health Insurance Card(s)
- Diagnostic Testing Results – X-Rays, CAT Scans, MRIs
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Failure to contact our office prior to your missed appointment will result in a $25.00 fee. This fee will be your responsibility and must be paid on or before your next visit in order to see one of our providers.
Insurance Information
We participate with most insurance plans, but it is your responsibility to call your insurance company to determine whether or not we participate with your particular plan. Please be prepared to provide your insurance card at every visit. Also, tell us if there is a change in your address, telephone number, or insurance coverage at the time of your visit.
Patient Billing
We will bill all primary and secondary insurances for you, if you provide the insurance information. Co-payments are expected to be paid at the time of service. We accept cash, checks, Visa, MasterCard, Discover and American Express. To make a payment, please visit click the link above to Pay Online or call the office.
Medications & Refills
Prescription refills are considered Monday through Thursday from 8:00pm to 5:00pm and Friday from 8:00am to 1:00pm. We will not be able to refill on weekends or holidays, so please anticipate your needs and call during our office hours Monday through Friday. For patients residing outside of the Panhandle region we are unable to renew your prescription pain medication needs. Dr. LaGrone will be glad to work with your local physician to help coordinate your medication requirements.
